The scheme covers the cost of the medications, surgeries, treatment, diagnostics, and consultations up to Rs. 1.5 lakh per annum for a family. The coverage under the scheme is provided on a family floater and individual basis.

    2. MH - Aadhaar Card
    1. Income Certificate from Tehasildar
    2. Latest Electricity Bill
    3. Medical papers
    4. Pan card

All documents need to be submitted directly to hospital.
Application form is not needed as hospital has its own prescribed format and procedure.
